Measure #134 Preventive Care and Screening: Screening for Depression and Follow-Up Plan

MIPS(Merit-based Incentive Payment Systems) was designed to tie payments to quality and cost-efficient care, drive improvement in care processes and health outcomes, increase the use of healthcare information, and reduce the cost of care.  Each year CMS reviews the current measures and eliminates measures that have topped out or in their opinion have met their goal and changed behavior in the provider to routinely include the measure in their standard of care.

Systems4PT monitors changes in the MIPS program and in all participating clinics’ compliance with MIPS, to ensure the clinics receive any bonuses available. This year CMS removed measure #154 Falls Risk Assessment.  It also capped Measure 155: Falls Plan of Care and Measure 182: Functional Outcome Assessment at 7 points maximum.  (75 points and reporting of a minimum of 6 measures is required to obtain bonus).  There is also a 20 case minimum reporting requirement to receive the full points available and for many clinics achieving that 20 case minimum is difficult to achieve for Measure 126:  Diabetes Mellitus: Diabetic Foot and Ankle Care, Peripheral Neuropathy – Neurological Evaluation and Measure 127: Diabetes Mellitus: Diabetic Foot and Ankle Care, Ulcer Prevention – Evaluation of Footwear.

To assure that all of the Systems4PT clinics that are participating in MIPS this year can meet the requirements for the bonus, we added a 7th measure that is applicable to physical therapy practice and eligible for 10 points if criteria are met.  The measure requires the use of a standardized tool to identify people at risk of developing depression.  Systems4PT has added the PRIME MD-PHQ2 questionnaire to the patient portal personal health screen.  We have also provided auto-filling documentation necessary to report on the result of these two questions and eliminate the need for extra typing by the therapist.  The CPT codes to report on this measure are also programmed to appear on your flowsheets based on the results of the depression screen.  The full description of this quality measure can be found at the link below.

A lot of programming goes into adding these measures.  The programming is then tested, re-tested, and beta tested with select clinics.  Once it has passed all of these tests we release the update to all Systems4PT clinics.
